To request funding, please complete the following:

  1. Submit a written request for funding to each Commissioner.
  2. Complete the Non-Profit Funding Request Form which requires submission of the following documents:
  3.      • 501(C)(3) Tax Exempt Status Letter
         • Articles of Incorporation & Amendments
         • Signed Bylaws
         • Signed W-9 Form
         • Board of Directors
         • Most Recent Financial Statement
         • Most Recent Audit (if available)
         • Current Budget
         • Most Recent IRS Form 990 
  4. Submit the completed Non-Profit Funding Request Form and required documents to the County Commission Office, Attn: Theresa LeGrady.
  5. If you wish to make a presentation before the Commission, contact your County Commissioner after completing the previous listed steps.
